We are very proud of your student’s desire to serve on the Team Kids Leadership Team at his or her school.

To honor your student’s service and commitment to helping others, we would like to offer him/her a free student membership to Team Kids.

The benefits of student membership are:

  • Email notice of opportunities to participate in service projects throughout the year.
  • Team Kids will keep track of service hours for all service projects, including the school-based Team Kids Challenge program.
  • When enough hours are accumulated, Team Kids will order and present student with the Presidential Youth Service Award

In consideration of being allowed to participate in any way in any activity involving or sanctioned by Team Kids, Inc. a California non-profit corporation, its assigns, licensees, and legal representatives (“Team Kids”), I do hereby warrant that my child (“Child”) is a minor, and I have authority to act on his or her behalf. I hereby consent to the assignment and granting to Team Kids of the name (or any fictional name), picture, portrait, photograph, likeness, and/or voice, of my Child, in all forms and media and in all manners, including composite or distorted representations, for advertising, fundraising, publicity, promotional purposes, trade or any other lawful purposes, and I waive any right to inspect or approve the finished version(s), including a written copy that my be created in connection therewith. Accordingly, I and/or my Child hereby release and discharge Team Kids and persons acting for, or on behalf of, the foundation from any liability by virtue of the use of the media described herein.
